Add 4 extra Fields to a specific item in a form

Hello
I have a form to record assets (Equipment) by serial number.
I have special device that doesn’t have a serial number because it consisted of a bundle of 4 different devices, each one with its own serial number.
I wanted that every time we add this type of equipment, another 4 additional fields appear to insert these 4 serial numbers.
The type of equipment comes from a TYPE column, and each of the additional 4 serial numbers has to have a dedicated column for each one, not a description field where I can add all the serial numbers inside.

It is possible?

But I need to manually add 4 more columns to my Google sheet right?
And then use this expression in the Type Column, like everytime we choose this Type, appear all the 4 columns, otherwhise this 4 columns are hidden?
