Add a new row to another table using values from this row not working

Hi, 

I've searched all the other posts related to this and have not found a solution. I have an attendance app and the hitting the check-in button no longer seems to populate in the attendance log table.  The correct action and table are selected but the data just isn't populating. Any help would be greatly appreciated.
