Adding ref to form

I would like to add a ref table to a form.  What I currently have is a form that techs fill out with PM's on machines, I have a table that shows the months that the PM's are due.  This table is currently Ref'd and visible from a list of all the companies. What I would like is when the tech selects the company in the form then saves it, when they go back into the form they are able to see that info.

I copied over the formula from the other page that currently displays this info but it does not appear to be working.
