App logo missing on new iOS installs. Create shortcut missing from menu.

Hello World,

The icon that used to be present when adding a shortcut is now missing in addition to the "Add shortcut" option in the menu.

Confirmed present in brand and delete from the device,  shutdown restart power cycle of the device.

Thoughts on a solution?
