App works for other user in preview but not on phone

hi , 

I have a simple app to track health essentials for me and my wife. The app works on my phone but it gives me an error "app did not load successfully please contact the app creator" on her phone. When i try the preview functionality using her email it works but not on the actual phone.

Any ideas on what could be wrong?
