Automation gone wild

I have an automation that creates a ne row in 2 different tables when a row in a separate table changes to match the condition.

I ran it today and now the automation is continuously adding a new role to the 2 tables nonstop. Ive gone ahead and deleted the automation but it continues to occur.

Help!
