Data: Add a new row to another table using values from this row is not working

Hii all,

I have a use case wherein I have tables "Purchases" and "Purchase Inventory Transaction". How it is working is, I create a record in "Purchases" table and in the form, add related "Purchase Inventory Transaction", which can be multiple items. So basically, this inventory table is linked to the purchases table.
I have an action setup for Data: Add a new row to another table using values from this row, where whenever a new purchase record is created (including therein purchase inventory record), copy some data for the rows on "Purchase Inventory Transaction" sheet to a third table, "Inventory Balance". 

This action does not seem to be working, as no records are being added for any items being added to the "Purchase Inventory Transaction" table. I have attached the action to the Purchase Inventory Transaction_form.
