Drop down list to ifs

Can some one tell me why I'm not able to see this column ( follow up) in my app. Or can any one suggest a better option

The app formula is

โ€ƒIFS([Follow up]= "<1 week", DATE([Date of examination])+ 3,
[Follow up]= "1 week", DATE([Date of examination])+ 7,
[Follow up]= "2 weeks", DATE([Date of examination])+ 14,
[Follow up]= "3 weeks", DATE([Date of examination])+ 21,
[Follow up]= "4 weeks", DATE([Date of examination])+ 28)

What column is that app formula for? Please provide a screenshot of that column's configuration screen.

Is it ok.. 

Your configuration doesn't make much sense. Your App formula expression is trying to use the Followup column value to change the Followup column value (i.e., to change its own value). The column's type is Date, but the App formula expression compares the column's value to Text values. I can't figure out what your intentions are.

Please describe in plain language what you are trying to accomplish.

Sorry Steve for troubling you

I'm a doctor with only basic computer knowledge. 

I came across appsheet and was impressed that anyone from non-technical back ground can make working apps. 

I'm trying to make an app to record patient data upon hospital visit (eye) 

I'm stuck in 2 places 

1) I want to add text 'clock hrs' to number selected.. 

This is the extent of lesions which is spoken in clock hrs

Like if I selected the outcome should be 1 clock hrs. 

2) for patients review when I select

 1 week outcome should be today + 7

2 week = today+14

So on..
