Form View and Table View Design

Hi There,

I am creating an app with existing Oracle database. In the database for a particular site there are 10 records that are of 10 different questions. I need to design a form that'll ask those 10 questions and store them row by row. And I also want to see a single row in Table View which contains all those 10 questions.

Can anyone suggest me how can I do that?
