Form view puts column number next to each display name

Hi all

I can’t figure out how to show the “display name without the column number in front of it.

So in the Form View I’ve attached, I want 

“Vine number”

instead of “2. Vine number”. How to remove the “2.”?  Same for the other fields.

You can hide that number from Editor > Settings > View type options > Forms > Hide form numbering
