Isblank action for a series of yes/no columns

I wanted to have an action that would create a N/A if someone failed to select yes/no. I have a checklist with roughly 30 questions as yes/no. I have Showif on each of those if further documentation is required for a yes or no response. I am trying to create a blanket isblank() for all those columns.

ie:

[Work Boots] Yes/no

[Corrective Action] showif[work boots]=false

Now I want an Action that if [work boots] is left blank instead of Y or N it spits out N/A

The best option I can think of is a column of type Show. You would need a separate Show column for each potential N/A.
