Key? field not visible in form view

I have made a form view ,Now I have made a form field "ID" as a key for this table but on my app when i open form that ID field is not displayed and I have also used UNIQUEID() in the "ID" field in form but that is also not working..

Where have you given the UNIQUEID() expression. Please post a screenshot of Table if possible

Have you specified any Column Order for this form view?

What am I supposed to be looking at in these new screenshots? Only thing I see is that now the id is not showing up in your emulator, so what is the difference between these screenshots and your first ones?
