Where to go to make changes to my template? I forgot to add a field and the app is already deploye

Hi,

I made an app and would like to add a field to a form but I can't find the way to do this. FYI the app is currently deployed but no one is using it yet until I make the change. Please advise where do I need to go to add this field?
