how to add multiple comments against single form

Hi , I  have created a from which has multiple columns with Enum field .I  want to add  multiple  comments against a single form how to do that ?

Do you mean you would want users to add a separate comment for each enum column?

If so, in general you could add a separate long text column for each enum column. So if there are 5 enum columns, there would be 5 comment columns of long text type.

If you would like one consolidated comment column for the entire form, you could  add one long text column, preferably at the end of the form.
