Help button for form questions.

It would be nice if the questions for a form could include a small help button, in the format of a small questions mark or an "I" button. This would reduce the need to include so much text into a question, creating a better UI for the apps. You could simplify the questions down to a few words and then have a small help button that when pressed, will give you a pop up box with a short detail on what the questions is asking. As an example I have attached an image.

Your screenshot of example is broken, but I get your point.

In fact, I wish the Description field in the Display section will act as a help tooltip option instead of replacing the displayed question. 
Then there should be an option to either show this only as a tooltip, or permanently show it as a <small> section below the question.
