Adding attributes to a Particular entity and evaluating the attributes

Hi,
I had a problem statement and I really stuck in solving this. There should be a way to attach KPIs related to a Particular Project while creating.
These KPIs need to be evaluated(scores) on a daily basis
That means different Projects with applicable KPIs and these need to be evaluated.

Ex: 1. a. Project_form: Name: Project A
Applicable KPIs: KPI1, KPI2, KPI3
1. b. Project_form: Name: Project B
Applicable KPIs: KPI4, KPI5.
2.a. Evaluating Project A form:
Name: Project A
KPI1 : user enters value( Like YES, NO, NA)
KPI2 : YES
KPI3: No
2. b. Evaluating Project B form:
Name: Project B
KPI4 : NO
KPI5 : YES
