Auto-Fill fields when another previous field dropdown on same form is selected

Hi, How do I auto-fill fields on a form when another previous field dropdown on same form is selected? For example, I have a separate table called associates with the columns Associate Name, Email, and Phone. On a form, I’d like the individual to select an Associate name in a dropdown and have that auto-fill the Email and Phone before the form is saved.
