Beginner Question

Hi - I’m looking to use appsheet to prototype an app which collects data from customers based on their preferences. I’m looking to find a way to have sections of questions based on simple things… age, true/false, zip code.

Does anybody have an example I can use? Been going around YouTube and don’t see anything.

Help would be appreciated.
