How do I print names on certificate, from rows of names in Google Sheet?

I use Google Forms to collect some information including name, phone number, date of training, email etc. I want to print names, date of training and certificate number, from the data collected with Google Form, on already designed hard paper certificate. Please is this possible with AppSheet? How do I do it?

I presently use Google Doc to print the information on the certificate, so I have to manually type the information on the blank Google Doc page before printing. This takes a lot of time to I would like to automate it with AppSheet.
