How do you get the first, second and third input into the pdf template?

Hii , need help , how do you get the first, second and third input into the pdf template?
because when I do input it always appears in the first line, and the previous data is overlapped

iam input in application appsheet ,
but the data that appears is always the updated one, the old one keeps disappearing
