How to show image in a question?

Hi, I’ve a selling app. I want to show an image of every product in Form UX. Also write name of the product beside the image.

Can someone kindly share the whole procedure with me? There’s a screenshot below, how i want my app to look like

• Tick “Label” option on the image column and name column of your items table
  
• this looks like a Deck view type
