Multiple Yes/No checks per Form question

I have forms that require three people to tick Yes/No for each question. But I can only get it to show one check.

How is this done?

Do you mean you should have Yes/No buttons for all people but in the same field? Meaning 6 buttons?

Instead of using Enum, try to use EnumList if that would work in your case.
