Weโ€™re trying to do some forensics to determine what happened. It appears that there were about 200 total requests that hit issues over the course of a couple of hours.

FWIW - It is possible the behavior was different for someone already signed in vs someone signing in from a new computer/browser. When I re-authenticated on a different machine, I got in.
