Same here. For me, the Form Saved action is not executing for forms whose data source is a slice.

Yes that seems to be the case for me also. I think it actually started last night.

We found the source of the problem and have released a fix. You can get the fix by refreshing the page if running the app in browser, or by syncing and then fully closing and then relaunching the app if on mobile. Sorry for any disruption this caused.

It looks like the problem was specific to forms based on slices. I’ll make sure we have a test case for form event actions in that scenario.
