New Bug Encountered: Form Disappears When Data Edited

In my app, I have a table that contains data with dates and names based on another data set. However, when I go to hit edit in one of the records, the form disappears from view. I have the same issue when I go to enter a new record. I can enter the first box but as soon as I tab to the next box the form disappears. Any ideas on how to fix this or what might be the problem?
