Quick question, want to find a sample give permission starting screen

In the past, when I’ve downloaded some of the sample apps, the screen has info like john@appsheet.com will get certain info about you… It’s a disclaimer type of screen.

I need a screenshot of one of these and can’t seem to find an app with that.

If you have one? Could you attach a screen shot? Or tell me how to make one for my app?
