To open Form depending on a value

Hi community it’s me again :),

I have a truck Inspection App that the drivers use on a daily basis to report any issues found while doing the truck inspection. The checklist have got 2 options “Issue” and “No Issue” as shown below:

Is there a way to open a form (Repair form) automatically when the driver click “Issue”? so he/she fills the form to request the respective repair and once the form is saved, come back to the inspection checklist form?

I am not sure whether there is a way around to achieve this. Please let me know your thoughts.
