Using Data to Complete Existing PDF Form and E-mail

Greetings,

I want to use data that is input into a survey tool, to then fill in a number of fields in a PDF form. I’m familiar with the Word doc format, and can pull the data fields into it correctly, such as <<[NAME]>>. However, I’ve tried to use the form template as an image, and then overlay a text box with those fields to no avail. I would use a custom template, however the output needs to be in a standard format that is used by a 3rd party. Looking to use the output of the survey to fill in the fields below.

There is no way to use an existing PDF as a template for AppSheet.

Not really sure is this is what you mean. But if you mean the image above then you’d need to add a table to your Word or Google Docs template file
