Adding Disclaimer in form

Hello All,

I am looking to use add disclaimer during the registration by using form. Currently , I am using the form to get user registered and want to add “disclaimer” after which a check box and after which user allowed to gain App access.

Check out the Show (Text) column type; I frequently use these inside my forms to literally “tell people what this form is for” - instruction text.

Thanks , but how this can be achieved with the check box along with the text which describe disclaimer.
