One form, two rows? Possible?

Hi everyone,

Im trying to set up a form that would create two rows on the same table.

Basically what I´m doing is setting up repeating tasks that have to be completed, depending on a column value [Future Date].

For example, in the form I have completed a task, and want to program the same task in a week from now. For this I have a column in that form that allows to select this future date.
But I cant use this row for a Report or WorkFlow so that it reminds about the “programmed activity”, because it has no “task completion” values for the “programed activity”, it needs to be an independent row for that as far as I understand.
That way if its “not completed” it will be included in the report.

Any ideas for this?
